Coventry

This service is for you if you are aged 16 plus, and are looked after by, or are leaving the care of Coventry Council, whether you live in the borough or not. We manage the services on behalf of the Council and have done so since April 2009.

This service is for you and we want to know what you would like to see. We would be delighted if you would join the Chances young people’s group, or the other groups that take place at the Aftercare Service, to shape how we can help you better.

Under the Children (Leaving Care) Act 2000, you have a right to support and care until the age of 21 or, if we are supporting you on a planned course of education or a program of training, we will support you until you finish the course.

We will:

• provide you with a Personal Adviser to provide advice and support
• contact you regularly to see how you are doing
• work with you to draw up a Pathway Plan mapping out your hopes and plans for the future, and how you are going to work towards that future
• provide financial help with living expenses up until you are aged 18
• help with further education

We relate our help and services to the government’s “5 Outcomes”, which are the outcomes that young people have said are important to them. In line with these outcomes, we will support you to try to ensure that you can: stay safe; be healthy; make a positive contribution; enjoy and achieve; and achieve economic well-being.
